  • Prompting clients to take medication from original packaging or a monitored dosage system.
  • Checking the care plan and medication record before support is given.
  • Recording prompts, refusals, omissions, or concerns after the visit.
  • Reminding clients about prescribed creams, eye drops, or other agreed routines where included in the plan.
  • Alerting family, the office, pharmacy, GP, or emergency services where the care plan requires escalation.

Important boundaries

Care workers do not change medication, give clinical advice, or make decisions about doses. Any uncertainty is escalated so the right professional or nominated contact can advise.
